Instructions for applying to the INPUT program:

Please read these instructions carefully and gather the information needed to complete the application.  Only completed applications will be considered. Incomplete applications will result in a delay of reviewing.

There are four sections for the INPUT program application:

  1. Demographics
  2. Academics
  3. Statements
  4. Letters of Recommendation and Signature
  • Once you have submitted your application, no changes can be made.

  • Your full legal name (First, Middle, Last)
  • Date of birth (mm/dd/yyyy)
  • Your valid University of Minnesota or other email address
  • Your current gender identity (e.g. female, male, trans-, non-binary, other, prefer not to answer)
  • Your preferred pronouns (e.g. she/her/hers, they/them)
  • Languages that you speak, including English. You must indicate as least one language.
  • First Generation: Are you one of the first in your family to attend college or university?
  • Childhood City, State, and Country: Where you spent the most time as a child growing up.
  • Family Income Level
  • Number of people in your immediate family (household)
  • Disability as defined by the Americans with Disabilities Act
  • U.S. Veteran status.
  • School, College, or University. You must be a currently enrolled student at a recognized accredited institution.
  • Your major. (If you have not declared a major, write "undeclared").
  • Your minor. Write "none" if you do not have a minor.
  • Your year in school (Freshman, Sophomore, Junior or Senior) at the time of application.
  • Overall Grade Point Average (GPA). Please use the 4.0 scale. Include only the courses that you have completed.
  • Biology, Chemistry, Physics, Math (BCPM) GPA. Please use the 4.0 scale. Include only the science courses, including physiology if applicable, that you have completed.
  • What bio-science topics are interesting to you?
  • Why are you interested in a research experience?
  • How does physiology relate to your career goals?
  • Please describe a situation where you felt that you were at a disadvantage.  Include how or whether the situation was resolved.
  • Use the space below to provide our selection committee with any other information that you feel makes you a good candidate for the INPUT program.

NOTE: You must compose your Personal Narrative or Statement in your word processor and then copy and paste your narrative or statement in the box provided.

  • Letter of Recommendation: You must provide the names and email addresses of two recommenders. These can be work or volunteer experience supervisors, current or former teachers, counselors or research advisors.  Think of someone who knows you well.
  • Signature: By signing and submitting this application you certify that the information you have provided in this application is truthful and to the best of your knowledge.
  • Contact your recommender to send the letter directly to [email protected] with the subject line to include your name and 'INPUT'.

When you have completed your application, click submit. You may apply only once.
